Terms of Service & Cancellation Policy

These Terms of Service (“Terms”) govern your use of the website and consultation services provided by Jacqueline Sutton, FNP-C, operating as Integrative Beauty & Wellness (“we,” “our,” or “us”). By accessing our website or purchasing any service, you agree to be bound by these Terms. Please read them carefully before proceeding.

1. Nature of Services

We provide telehealth consultation services only. All fees paid to Integrative Beauty & Wellness are exclusively for professional consultation time, clinical assessment, wellness protocol guidance, and ongoing monitoring by a licensed healthcare provider.

We do not sell, dispense, ship, or fulfill prescriptions or medications of any kind. Any prescriptions that may be issued as part of your clinical care are separate from our consultation fees and are filled independently by you at a licensed pharmacy of your choice. We have no financial relationship with any pharmacy in connection with your care.

Our services include:

• Initial comprehensive health consultations (60 minutes)
• Monthly ongoing wellness consultation packages
• Health optimization protocol design and monitoring
• Lab work review and coordination guidance
• Lifestyle, nutritional, and wellness guidance
• ProxiGene™ Genetic Panel interpretation and guidance (one-time add-on)

2. Eligibility

You must be at least 18 years of age to use our services. By engaging our services, you represent and warrant that you are at least 18 years old and have the legal capacity to enter into these Terms. Our services are intended for individuals located in states where Jacqueline Sutton, FNP-C, is licensed to practice. We will confirm eligibility during your initial consultation.

3. Fees & Payment

Fee Schedule

• Free Discovery Call: 10-minute complimentary consultation — no charge
• Initial Comprehensive Consultation: $99 (60 minutes) — one-time, non-refundable after the consultation has taken place
• Monthly Wellness Consultation Packages: $450 per month — covers all consultations, monitoring, and clinical guidance for that billing period
• ProxiGene™ Genetic Panel: $700 one-time fee — covers panel interpretation, report, and clinical guidance session

Payment Terms

All fees are due at the time of service or at the start of each billing period. Payments are processed securely via Stripe. By providing payment information, you authorize us to charge your payment method for the services selected. Monthly packages are billed on a recurring monthly basis beginning on the date of your first monthly package payment.

All prices are listed in US dollars. We do not accept insurance for any of our services. Some services may be eligible for HSA/FSA reimbursement; please consult your plan administrator.

4. Cancellation Policy

Monthly Packages — Cancel Anytime

Monthly wellness consultation packages may be cancelled at any time with no penalty. To cancel, contact us at info@jlsintegrativebeauty.com at least 5 business days before your next billing date to avoid being charged for the following month. Cancellation requests received fewer than 5 business days before the next billing date may result in one final charge for the upcoming month.

Upon cancellation, your access to consultation services will continue through the end of the current paid billing period. No partial-month refunds are issued for mid-cycle cancellations.

Individual Appointments

We ask for at least 24 hours’ notice to reschedule or cancel an individual appointment. Appointments cancelled with less than 24 hours’ notice or missed without notice (“no-show”) may be forfeited without refund.

ProxiGene™ Genetic Panel

The ProxiGene™ Genetic Panel fee is non-refundable once the panel has been ordered and processing has begun. If you cancel before ordering has commenced, a full refund will be issued.

5. Refund Policy

• Free Discovery Call: No charge; no refund applicable.
• Initial Consultation ($99): Refundable only if cancelled at least 24 hours before the scheduled appointment. Non-refundable once the consultation has taken place.
• Monthly Packages ($450/month): Non-refundable for the current billing period once the period has begun. Cancel before the next billing date to avoid future charges.
• ProxiGene™ Panel ($700): Non-refundable once panel processing has commenced.

If you believe a charge was made in error, please contact us within 30 days at info@jlsintegrativebeauty.com and we will review your case promptly.

6. No Provider-Patient Relationship via Website

Browsing this website or reading its content does not create a provider-patient relationship. A provider-patient relationship is established only upon completion of a formal intake process and mutual agreement to proceed with care.

7. Telehealth Consent

By scheduling and participating in a telehealth consultation, you acknowledge and consent to the following:

• Telehealth services are delivered via secure video platform and involve the electronic transmission of your health information
• As with any medical consultation, there are potential risks, including technical failures, limitations of remote assessment, and privacy risks inherent to electronic communications
• You have the right to withdraw from telehealth services at any time and request in-person care from another provider
• Telehealth is not appropriate for emergencies. In the event of a medical emergency, call 911 or go to your nearest emergency room immediately

8. Medical Disclaimer

The information provided on this website is for educational purposes only and does not constitute medical advice. Consultation services are intended to support, not replace, your relationship with your primary care provider. Results from any wellness protocol may vary by individual. We do not guarantee specific health outcomes.

This practice does not diagnose, treat, cure, or prevent any disease within the meaning of FDA regulations.

9. Limitation of Liability

To the maximum extent permitted by applicable law, Jacqueline Sutton, FNP-C, and Integrative Beauty & Wellness shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services or website. Our total liability for any claim arising from or related to our services shall not exceed the total amount paid by you for the specific service giving rise to the claim in the three months preceding the claim.

10. Intellectual Property

All content on this website, including text, graphics, logos, and educational materials, is the property of Jacqueline Sutton, FNP-C / Integrative Beauty & Wellness and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, or create derivative works from any content on this site without our prior written consent.

11. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the state in which Jacqueline Sutton, FNP-C, holds her primary license, without regard to conflict of law principles. Any disputes shall be resolved through binding arbitration or in a court of competent jurisdiction in that state.

We reserve the right to update these Terms at any time. Changes will be posted on this page with an updated effective date. Continued use of our services after any changes constitutes your acceptance of the revised Terms.
